Program Fee Refund Policy

Refunds or credits will be given if the payment is being used for the patient’s personal account.  Payment for services provided is not refundable.  You must complete a Request for Refund form to request a refund and the refund if appropriate will be made within thirty (30) days of the receipt of the Request for Refund form.
